Waymark deploys local teams of community health workers, pharmacists, therapists, and care coordinators who leverage proprietary data science technologies to deliver evidence-based interventions to hard-to-reach patient populations.[3] The founding team includes Rajaie Batniji (Co-Founder & CEO, previously Co-Founder of Collective Health), Sanjay Basu (Co-Founder & Head of Clinical, former Internal Medicine Physician at HealthRIGHT 360), Afia Asamoah (Co-Founder, Head of Legal & People, former Head of Legal, Google Health), Brett Hickman (Head of Partnerships, former Chief Commercial Officer at Modivcare), and Sara Greenbaum (Head of Care Operations, former Regional Market President at Eleanor Health).[3] Waymark has raised $45 million from investors such as Andreessen Horowitz, New Enterprise Associates, and Lux Capital.[3] In a Pennsylvania Medicaid trial, Waymark’s approach led to a 12% higher quality rating, a 24% reduction in repeat admissions, a 17% reduction in 30-day readmissions, and a 30% reduction in hospitalizations.[3] The company differentiates itself through its community-based care model, proprietary data science, and focus on Medicaid populations.
